The Home Health Aide (HHA) is responsible for providing hands-on personal care, simple procedures as an extension of therapy or nursing services, assists in ambulation or exercises, assists with activities of daily living and medication reminders. Abides by and follows the established policies and procedures of the Agency and State and Federal regulations.Key ContributionsPerforms only those personal care activities written per patient care instructions by a registered nurse or other appropriate skilled professional.Participates as a member of the interdisciplinary team and provides services that are permitted to be performed under state law and are consistent with home health aide training.Assists the client with personal care, simple procedures as an extension of therapy services, such as ambulation and prompting for exercises, household service essential to health care at home and assistance with medication reminders.Reports changes in clients' conditions to the Case Manager or Clinical Supervisor.Competent in completing appropriate documentation in compliance agency policies and procedures.Demonstrates respect for client, facility, and staff member property.Maintains a clean, safe environment.Recognizes emergencies and has knowledge of emergency procedures.Reviews clients' care plan prior to performing any care. Documentation reflects care plan is being followed.Attends in-service classes and staff meetings. Completes annual mandatory updates by deadlines.Completes continuing education units annually as mandated by state regulations.Works with confidential data, of which disclosure would be contrary to the best interest of the residents, the facility, and professional ethics. Actively promotes resident confidentiality and rights. Works in compliance with all HIPAA regulations and standards.Performs only those personal care activities contained in a written assignment by a qualified health professional employee, which shall include but shall not be limited to assisting the client with personal hygiene, ambulation, eating, dressing and shaving.Ensures that the client receives adequate nutrition including preparing the client for meals and assisting the client as necessary. Encourages and offers fluids to provide adequate oral intake.Assists the client in meeting the needs for bowel and bladder elimination. Provides incontinence care as needed. Collects urine, stool, and sputum specimens as directed by the nurse/M.D.Demonstrates respect for client, facility, and staff member property.Qualifications & RequirementsExcellent oral and written communication skills in the English language required.During emergencies or natural disasters, may be required to remain on duty for the duration of the event.Highschool diploma or equivalentFlorida State Nursing Assistant or Home Health Aide CertificationCompletion of written competency test with a minimum score of 82% and skills competency test with no errors is required.Valid state of Florida driver's license and proof of insurance.Previous experience in home health, hospital, or nursing home preferred.
