Community Health Manager: Lead Homeless Outreach & THRIVE

City of Redmond is seeking a Community Health Manager to lead the Homeless Outreach team within the THRIVE: Community Health Program. You will guide outreach, coordinate care plans, and work with police, fire, and social service partners to deliver coordinated crisis interventions. The role emphasizes staff development, data-informed decision making, and strong communication with City leadership.
