Community Health Manager

As the direct supervisor for the Homeless Outreach team, this role provides steady, supportive leadership while also coordinating cross-department efforts within the City’s innovative THRIVE: Community Health Program. The Community Health Program includes the Homeless Outreach, Mobile Integrated Health, and Mental Health co-response team, whose goal is to provide safety, stability, opportunity, and hope for anyone in need or crisis. With a focus on guiding staff, fostering collaboration, and ensuring alignment with broader departmental goals, the Community Health Manager helps create responsive, effective strategies to meet community needs and helps ensure the Community Health teams are providing cohesive, coordinated, and effective response to community needs and working together seamlessly. Supervise and guide the daily activities of Homeless Outreach Team to ensure effective outreach, intervention and follow-up services are provided to individuals who are unhoused or at risk of losing their housing. Leads aspects of the City’s THRIVE program which consists of homeless outreach, community court, human services, mobile integrated health and mental health co-response. Coordinate efforts among the planning, police, and fire departments, as well as community and regional partners, to support behavioral health, homelessness response, chronic illness, and other crisis-related services. This position plays a critical role in ensuring clear communication and information-sharing across all THRIVE program partners. The Community Health Manager is responsible for maintaining up-to-date awareness of client needs, staff workload, emerging issues, and follow-up tasks, and for making sure this information is shared promptly with internal departments and external service partners. By providing consistent updates, tracking progress, and facilitating collaboration, this role ensures that outreach efforts are aligned, gaps are identified early, and teams can respond cohesively to support individuals and families in crisis. Supervise and guide the daily activities of Homeless Outreach staff to ensure effective response. Provide guidance and support to Community Health teams handling mental, behavioral, social, or emotional crises. Lead case-conferencing to collaborate on the development of recommended care plans, resource placement and follow-up strategies, and handle complex cases. Maintain resources for mental health, behavioral health, homelessness, and other needs, actively seeking out new resources that will support the Community Health teams’ goals. Guide Community Health team members on appropriate referrals and continuously evaluate resource effectiveness by tracking client outcomes. Recommend and implement updates to policies and protocols to strengthen a coordinated response. Ensure staff compliance through coaching, guidance, and clear communication, and address performance issues by outlining necessary corrective actions. Maintain communication with stakeholders, including service providers, community organizations, and City departments. Oversee documentation of client interactions, assessments, and service plans to ensure compliance with standards. Develop reports and provide regular updates on team activities, resources, and outcomes to City leadership. Oversee scheduling needs, staffing, and case assignments to ensure adequate coverage and efficient use of resources. Demonstrates strong systems thinking skills to understand complex organizational relationships, anticipate downstream effects of decisions, and develop integrated strategies that advance long-term objectives. Represents the City at coalitions, partner meetings, and community groups regarding homelessness initiatives. On behalf of the Community Health program, coordinates and leads data collection and analysis and develops reports and presentations to City leadership and Council. Builds and maintains partnerships with human service agencies, community-based and faith-based organizations, jurisdictional partners, and regional outreach professionals. Knowledge of and skills in: Experience with mental health providers, healthcare providers, shelter providers, landlords, affordable housing providers, and other community programs which may support vulnerable participants. Experience in internal coordination with public safety departments, human service programs, and the Executive Office. Effective oral and written skills, including active listening and non-verbal communication. Ability to establish and apply data collection needs and standards, evaluate and synthesize data and trends from diverse sources, and present insights clearly and concisely. High cultural and emotional intelligence. Strong commitment to dignity and respect for all persons. Knowledge and experience working in East King County is preferred. Experience drafting and presenting presentations to elected officials and/or leadership teams Principles of supervision, training, and performance evaluation. Abilities to: Provide support, guidance, and advocacy to individuals in challenging situations and in crisis; Understand professional boundaries, ethical guidelines, and confidentiality principles. Exhibits cultural competence and a deep respect for cultural, ethnic, and diversity issues, dedicated to offering inclusive and responsive services. Establish and maintain effective working relationships with department and City staff, City officials and the public. Apply systems thinking to understand complex organizational relationships, identify interdependencies, and anticipate impacts across programs, policies, and operations. Resolve issues effectively and efficiently. Work closely and effectively with City leadership, elected officials and staff. Set priorities and direct, guide, motivate, and evaluate the work of subordinates. Plan, organize and schedule work assignments. Guide team members on proper work methods, processes, and procedures. Provides as needed back-up support, which may include providing direct outreach to people experiencing homelessness. Perform additional duties as assigned, including special projects, community meetings, and departmental initiatives as applicable. Education and experience: Master’s degree in social work, Psychology, or related field; two years’ experience working with individuals with mental, behavioral, and / or emotional health concerns; and at least one year of lead and / or supervisory experience Or a combination of education and/or training and/or experience which provides an equivalent background required to perform the work of the class Selected candidates are required to pass a background check. Licenses and certificates: Valid Washington State driver’s license required Licensing from the Washington State Department of Health such as Licensed Independent Clinical Social Worker, Licensed Advanced Social Worker, Licensed Mental Health Counselor or equivalency.
