In this role, the RNCM collaboratively works alongside an interdisciplinary team to provide end-of-life care to those needing hospice services in their home or community setting. The RNCM provides individualized case management, admission facilitation, clinical support, continuous care, after hours call, and/or other skilled nursing expertise with chronic and end-of-life care.
