Behavioral Health Case Manager & Care Coordinator

Hygea Healthcare is seeking a Case Manager to provide comprehensive case management for patients struggling with substance use and mental health challenges. Responsibilities include assessing patient needs, coordinating care, and collaborating with medical professionals to ensure effective treatment. The ideal candidate will have a Bachelor's degree in a related field and preferably 1-2 years of experience in behavioral health.
