Job Summary The Case Manager is responsible for complete case management of all patient needs. Interview patients and families to assess needs and risk categories to develop and implement appropriate plans of care. Ensure quality clinical care by coordinating all services and collaborating with doctors on the plan of care. Implement, monitor, and evaluate the plan of care and its effectiveness on a regular basis. Communicate regularly with relevant staff members and family members on care progress. Essential Job Functions Bachelor’s degree in Social Work, Counseling or related Human Service field preferred. Certified Peer Recovery Specialist certification preferred or obtain CPR certification within six months. 1-2 years of experience working in a behavioral health setting. Knowledge of health care, detoxification process, addiction, co-occurring disorders, DSM Criteria, and terminology. Ability to read and interpret written information; write clearly and informatively; edit work for spelling and grammar. Ability to speak clearly and persuasively in positive or negative situations; listen and get clarification; respond well to questions; demonstrate group presentation skills; and participate in meetings. Working knowledge of Microsoft Word, Excel, and Outlook. Core Competencies At Time of Hire Clinical Knowledge: Proficiency in case management, care coordination, and discharge planning. Communication Skills: Ability to clearly articulate treatment plans and progress to patients, families, and teams. Organization: Proven ability to manage multiple cases, prioritize tasks, and meet deadlines. Problem‑Solving: Demonstrated skill in assessing patient situations and providing effective interventions. Team Collaboration: Experience working as part of an interdisciplinary team to achieve patient goals. Attention to Detail: Accuracy in maintaining records and ensuring compliance with regulatory standards. Ethics and Confidentiality: Adherence to HIPAA and ethical guidelines in handling patient information. This job description does NOT constitute an employment contract.
