What You'll Do Provide scheduled and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing facilities. Assess patient needs and provide symptom management, medication education, wound care, and comfort-focused nursing interventions. Educate and support patients and families throughout their hospice journey. Collaborate with the interdisciplinary team to ensure individualized, compassionate care. Complete timely and accurate EMR documentation. What You'll Bring Active Minnesota Registered Nurse (RN) license. Hospice experience preferred, with consideration given to nurses from home health, oncology, ICU, palliative care, or other end-of-life care settings . Outstanding communication and relationship-building skills. Ability to work independently while managing a field-based schedule. Reliable transportation and a valid driver's license.
