As a leading children's health system experiencing steady growth, we're excited to expand our team with exceptional team members who share our vision of transforming children's health and well-being as one team.ResponsibilitiesPerform referrals and assists clients in linking with community support systems and agencies.Participate in administrative and clinical case conferencing for clients/patients with medical staff/medical case managers in order to address client's/patients medical and psychosocial needs.Performs data entry tasks for evaluation purposes.Monitor client progress within the health and community support systems.Provides education and training to residents/individuals in their respective community, works as part of the multidisciplinary team to prepare and implement plans, (example for HIV Pediatric and Youth program, Recovery Support Services; and general health awareness. Completes all required documentation.Participate in (in person/Virtual) community and planning meetings such as the Ryan White Planning Council and CT HIV Prevention Council, OCCH community meetings.Other duties as assigned.QualificationsMinimum Education and Experience Required:Education Required: High school diploma or GED.Experience Required: 1 year experience working in one of the following: Medical case management, community health experience, community organizing/mobilization, faith based community programs, or worked with or experienced children/youth/individuals impacted by social determinants of health or chronic illness/substance/recovery programs.Experience Preferred:Living and/or working in an urban setting.Ability to work with diverse client populationAbility to read and write; bilingual English/Spanish preferredKnowledge, Skills, and Abilities:Knowledge of: Community assistance resourcesSkilled at: Oral and written communications; interpersonal communicationsAbility to: Work with diverse client population, read and write (English/Spanish if required), Flexible schedule: work evenings/weekends as needed
