Hospice RN Case Manager

Part-Time RN Case ManagerAthena Hospice of Rhode Island is seeking a part-time RN Case Manager to join our dedicated team! As a Hospice RN Case Manager, you will be responsible for planning, organizing, and directing the hospice care plan for assigned patients under the direction of the clinical director. We offer a competitive hourly rate of $45 to $50, with weekly pay, and travel throughout Rhode Island is required.Responsibilities:RN completes initial, comprehensive, and ongoing assessments of patients and families to determine hospice needsProvide a complete physical assessment and history of current and previous illnessesRN supervises hospice aidesProvide professional nursing care by utilizing all elements of the nursing processAssess and evaluate patient status, writing and initiating a plan of care, and regularly re-evaluating patient and family/caregiver needsDevelop a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actionsRN provides counseling to patients and families in meeting nursing and related needsRequirements:Active and unencumbered RN license in Rhode IslandNursing degree from an accredited college, university, or schoolPrior RN experience in hospice is preferredAvailable for Eligible Team Members:Employer-Paid Life Insurance401(k) with Employer MatchVacation and Personal TimeHealth, Dental, and Vision InsuranceWe are an equal opportunity employer that values diversity at all levels.
