We are looking to hire a qualified and compassionateLicensed Vocation Nurse (Floor) for their Primary Care Department. Reporting to the Director of Operations, the LVN underthe direction of a physician, advanced practice provider, or Registered Nurse,delivers compassionate patient care and works collaboratively with thehealthcare team to promote health, prevent illness, and support positivepatient outcomes. Schedule: Full-Time | 1.0 FTE What You’ll Do Key Responsibilities Demonstratethe health center’s mission by providing quality care and serving as an LVN resource to the care team, as applicable Performnursing duties within scope, including wound care, immunizations, and patient health education Completemedication reconciliation and administer prescribed medications and treatments per written or standing orders Maintainup-to-date emergency kits and oxygen tanks Completerequired forms in accordance with health plan and program standards Performsall duties in support of successful EHR and patient portalmanagement Contactpatients to schedule appointments as needed Participatein care team and other assigned meetings Deliverservices in full compliance with the health center’s standards Respondto and appropriately escalated provider and patient messages using SBAR and established health center policies and procedures Communicatethrough messages with external providers and agencies regarding patientcare, issue resolution, and escalation Respondto patient portal requests for forms, labs, and test results withinscope Performother related duties as assigned What We Are Looking For License, Certification and Experience Highschool diploma or equivalent BLScertification required 1+year of LVN experience; outpatient clinic experience preferred Additional Skills and Attributes Abilityto work effectively in a fast-paced environment Professional,supportive, and team-oriented attitude What You Can Expect Fast-paced,collaborative clinical environment Regularinteraction with patients, providers, and healthcare staff Sharedoffice and patient-care setting Frequent communication, documentation, and patient support Abilityto manage multiple priorities while maintaining quality care Compensation and Benefits: CompetitivePay Medical,Dental and Vision Insurance HolidayVacation and Sick Leave EmployeeAssistance Program LifeInsurance (Basic + Voluntary) TuitionReimbursement 403(b)Retirement plan with 6% match #J-18808-Ljbffr The Staff Pad
