Position Project Manager, Care Coordination Employment Details Exempt, Full‑Time, Hybrid (3 days in office), Pay Range $90k–$95k, Open Date 2/17/2026, Shift: Days. Role Overview As Program Manager, Care Coordination in partnership with the Director and Manager of Care Coordination, you will develop and implement SSHCO Care Coordination models and workflows. You will create the overall workplan, track milestones across all SSHCO work streams, and lead implementation activities at all coalition sites. Key Responsibilities Develop work plans to achieve SSHCO project goals. Support project management processes and methodologies to ensure delivery on time and within budget, adhering to high quality standards and meeting key milestones. Maintain constant communication with leadership and project staff to ensure effective operations. Document and follow up on actions and decisions from meetings. Break projects into action items and create timeframes. Ensure projects adhere to frameworks and maintain documentation. Create and maintain comprehensive project documentation, plans, and reports. Prepare presentation materials for meetings. Ensure project deadlines are met. Monitor project progress and handle issues that arise. Assess project risks and issues and provide solutions. Incorporate stakeholder feedback into workflows. Identify and define requirements, scope, and objectives. Develop and implement care coordination workflows, processes, and protocols across participating hospitals and FQHCs. Organize, attend, and participate in stakeholder meetings. Assess and document existing care coordination, patient, and clinical workflows at coalition sites. Collaborate with coalition site staff to customize workflows for integration into existing operations. Support technology enablement (e.g., EMR, REDCap, new tools). Support quality improvement projects and initiatives. Ensure contractual obligations are met. Contribute to evaluation and dissemination of findings with SSHCO and community stakeholders. Measure and report overall model success by maintaining key performance metrics. Regularly update leadership on project status and barriers. Coordinate with subject‑matter experts, internal and external. Perform other duties as requested. Assist with special projects as needed. Organizational Responsibilities Create a positive, professional, service‑oriented work environment by supporting SSHCO’s mission and values. Work effectively as a member of the team. Maintain personal safety and report hazards immediately. Complete company trainings as assigned. Maintain strict patient and employee confidentiality in compliance with HIPAA. Use resources efficiently. Maintain professional licenses and continuing education credentials, where applicable. Travel Requirements Local travel to healthcare organizations on Chicago’s South Side may be required. Qualifications Bachelor’s or master’s degree in public health, business, health care administration, non‑profit management, or related field (required). Two to three years of project management experience (required). PMP certification encouraged. Two to three years overseeing population health management, care management, or similar program (preferred). Experience working with health systems, federally qualified health centers, CBOs, and/or collaboratives. Experience planning, implementing, and/or evaluating community‑based interventions. Non‑profit management experience. Knowledge of NCQA, Joint Commission standards and state/federal agency requirements. Strong business acumen and project management experience. Demonstrated problem‑solving, judgment, and recommendation skills. Accountability for polished deliverables. Ability to establish credibility and influence relationships. Excellent interpersonal, conflict resolution, and direction skills. Strong verbal and written presentation and communication skills. Ability to manage change and adapt as project needs evolve. Demonstrated commitment to racial and health equity. Passion for hands‑on problem solving and execution. Knowledge of quality improvement, data collection, and management. High proficiency with standard desktop and Windows‑based systems, including EMR (e.g., EPIC) and Microsoft Office. Ability to use other software as required. Ability to work with new applications and vendor software. Work Environment Hybrid work schedule. General office environment with moderate noise level.
