Social Worker – Long-Term Care

This is an exciting opportunity to become part of an established Social Work Department who work together to provide exceptional support, ensuring our residents receive the highest quality of care throughout their stay.

What You'll Do

• Advocate for residents' emotional, psychosocial, and discharge planning needs.

• Assist residents and families with admissions, care planning, transitions, and community resources.

• Coordinate safe and effective discharge planning.

• Participate in interdisciplinary care conferences and collaborate with nursing, therapy, physicians, and other departments.

• Support residents and families through difficult situations with empathy and professionalism.

• Maintain timely and accurate documentation in accordance with state and federal regulations.

• Help ensure compliance with all applicable long-term care regulations.

Qualifications

• Licensed Social Worker (LMSW or LLMSW) preferred , but experienced non-licensed candidates with long-term care social services experience are encouraged to apply.

• Previous experience in a skilled nursing facility or long-term care setting is strongly preferred.

• Excellent communication, organization, and problem-solving skills.

• Ability to prioritize and thrive in a fast-paced environment.

• Compassionate, positive attitude with a strong commitment to resident-centered care.
