RN Field Care Manager Nurse

The Field Care Manager Nurse 2 assesses and evaluates members’ needs to help achieve and maintain optimal wellness by guiding members and families toward resources appropriate for their care and wellbeing. Competitive pay, including eligibility for an annual performance‑based bonus Employee referral program Position Responsibilities Manage a caseload and complete assessments in home or community‑based settings, and telephonically. Provide clinical support and guidance for members with medical complexity. Help develop and coordinate care plans to ensure members receive appropriate services. Review, assess, and complete medical complexity attestations and clinical oversight activities. Ensure members receive services in the least restrictive setting, assessing their care needs. Develop and modify Individual Care Plans and involve applicable members of the care team in the planning process. Support members and/or caregivers using an interdisciplinary approach to access social, housing, educational, and other services regardless of funding source. Serve as the primary point of contact for the Interdisciplinary Care Team (ICT) and coordinate with members, ICT participants, and external resources to meet member needs. Required Qualifications Residence in Michigan (Wayne or Macomb Counties) Active Michigan RN or APRN license (including NP or CNS) with no disciplinary action 2+ years’ experience in health care and/or case management Ability to travel to homes and community settings for face‑to‑face assessments Experience working with the adult population in disease management Knowledge of community health and social service agencies and additional community resources Proficiency with electronic health records and other electronic information applications Excellent keyboard and web navigation skills Intermediate to advanced computer skills and experience with Microsoft Word, Outlook, and Excel Preferred Qualifications BSN degree Experience with in‑home assessment and care coordination Experience with health promotion, coaching and wellness Experience with Medicaid Long Term Care Previous managed care experience Bilingual – Spanish, Arabic or Chaldean Neo‑Aramaic Certification in case management Motivational Interviewing certification or knowledge Additional Information Schedule/Time Zone: Monday through Friday, 8:30 AM – 5:00 PM Eastern Time Zone Work Location: Michigan (Wayne and Macomb Counties) Work Style: Field Mandatory travel: 75–90% within Wayne and/or Macomb Counties Internet requirement for remote work: minimum 25 Mbps download, 10 Mbps upload; wireless, wired cable or DSL recommended; satellite or cellular only with approval TB screening required due to patient‑facing role Valid state driver’s license and personal vehicle liability insurance (minimum required limits or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 property damage) Eligible mileage: travel from home to first work location of the day, between client locations, and back home Interview format: HireVue video interview Language assessment: English‑speaking employees who converse in other languages must take a language proficiency assessment Pay and Benefits Pay range: $71,100 – $97,800 per year (based on geography, experience, education, and certifications) Bonus incentive plan based on company and individual performance Full‑time benefits include medical, dental, vision, 401(k), paid time off, paid parental and caregiver leave, disability, life insurance, and more Equal Opportunity Employer Humana is an equal opportunity employer.
