Community Case Manager - Behavioral Health & Court Support

The Department of Health and Social Services (DHSS) in Delaware seeks a qualified case manager to assess and coordinate services for specialty court clients under DOC supervision. You will conduct behavioral health assessments, refer clients to appropriate care, monitor progress, and report to the court via status hearings and reports.
