Position Summary

The Housing Navigator helps individuals and families experiencing homelessness or housing instability secure and maintain safe, stable housing. This position provides housing navigation, case management, and resource coordination from intake through housing placement and follow-up. Working with landlords, community partners, and service providers, the Housing Navigator removes barriers to housing and connects participants with the resources needed to achieve long-term stability.

Essential Responsibilities

• Guide participants through the Coordinated Entry process, including intake, assessment, referrals, and housing placement.
• Conduct assessments using HMIS and develop individualized housing stabilization plans.
• Provide trauma-informed case management and ongoing participant support.
• Assist with housing searches, rental applications, move-in coordination, and resolving barriers to housing.
• Develop relationships with landlords, property managers, and community partners to expand housing opportunities.
• Coordinate housing-related financial assistance, including security deposits, rental assistance, utilities, application fees, and moving expenses, in accordance with program guidelines.
• Connect participants to employment, public benefits, healthcare, behavioral health, financial literacy, and other community resources.
• Maintain accurate and timely documentation in HMIS and other required systems while ensuring confidentiality and compliance.
• Monitor participant progress and housing outcomes.
• Participate in case conferences, outreach efforts, and community collaboration.
• Perform administrative duties and other responsibilities as assigned.

Qualifications

• Bachelor's degree in Social Work, Human Services, or a related field, or an equivalent combination of education and experience.
• Experience working with individuals or families experiencing homelessness, housing instability, or other vulnerable populations.
• Knowledge of Coordinated Entry, HMIS, and housing assistance programs preferred.
• Strong communication, organizational, and problem-solving skills.
• Ability to manage multiple cases in a fast-paced environment.
• Proficiency with Microsoft Office and electronic case management systems.
• Bilingual English/Spanish preferred.
