Minimum Requirements:

• Associate's degree in physical therapy from an accredited educational program.
• Current Wisconsin PTA license.
• Experience working with older adults in skilled nursing or long-term care preferred.
• Documentation experience; familiarity with Net Health is a plus.
• Availability for day shift, Monday through Friday.
• Excellent communication and teamwork skills.
• BLS certification from AHA or ARC may be required.

Your Responsibilities:

• Provide physical therapy treatments under PT supervision to improve mobility, strength, balance, and function.
• Implement individualized treatment plans and support residents in achieving rehabilitation goals.
• Accurately document care and maintain compliance standards.
• Collaborate with therapy and nursing teams and participate in care planning.
• Educate residents and families on safe techniques to maximize independence.

What You Bring:

• Current Wisconsin PTA license.
• Strong clinical skills and a commitment to care for older adults.
• Flexibility, professionalism, and a positive attitude.
• Experience with electronic documentation systems; Net Health experience preferred.
