We are seeking a dedicated Physical Therapist to provide expert care to home bound geriatric patients. This role involves assessing patient needs, developing and implementing treatment plans, educating patients on their therapy, and closely monitoring their progress to ensure optimal outcomes.
Responsibilities
- Conduct thorough patient assessments to determine physical therapy needs
- Develop and implement individualized treatment plans for geriatric patients
- Educate patients and their families about therapy goals and techniques
- Monitor and document patient progress throughout the treatment process
- Maintain accurate and timely documentation of all patient interactions and therapies
Preferred Qualifications
- Licensed Physical Therapist
- Strong skills in patient assessment
- Excellent communication abilities
