Job Description
Title: LPN - Long-Term Care

Schedule:
- Shift: 10:30 PM - 6:30 AM
- Days of week: Every other weekend required

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 37.5 hours per week

Requirements:
- Active LPN license in the state
- 1+ years of experience in a long-term care facility
- Current Basic Life Support (BLS) certification
- Experience with PointClick Care EMR
- Skills checklist matching specialty required

Roles & Responsibilities:
- Provide nursing care and support to patients in a long-term care setting.
- Assist in the management of patient health assessments and care plans.
- Administer medications and treatments as prescribed.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Monitor patient progress and document care accurately.
- Participate in patient education and support family involvement in care plans.

Compliance / Notes:
- RTO (Request Time Off) must be included at the time of submission.
- Scrub color: Navy Blue
- Floating required within the same building.
