Job Description:

Under the leadership of the Applications Manager, the Application Analysts primary role is to have a complete working knowledge of assigned application, to include an understanding of the assigned department workflows for the associated system build/configure, upgrade and release process. The Application Analyst will act as the primary support contact for the application and coordinate issues that arise in their area of responsibility and must be very knowledgeable about the organization's policies, procedures, and business operations. They will perform workflow analysis, data collection and report issues associated with the software. The Application Analyst will serve as a liaison between team, end users, third parties and technical staff. They will work with end users by participating in training and troubleshooting issues and questions. Senior level technical professional. Completes complex work within a discipline or specialty area. Helps others understand the why and articulates the importance to others they work with. Owns processes and continually incorporates feedback to improve them. Proactively audits process to improve before an issue occurs. Caters to individual styles and abilities. Teaches new skills. Helps talk through situations with operational stakeholders. Makes recommendations for new functionality for issue avoidance or operation improvement. Pushes back when operations or IT is steering down the wrong path.

Specialization

The Epic Population Health Application Analyst serves as the primary application specialist responsible for the administration, support, optimization, and ongoing maintenance of Epic Healthy Planet and related Population Health applications. This role provides technical expertise in care management, quality improvement, value-based care initiatives, patient registries, and population health workflows that support organizational strategies for improving clinical outcomes, enhancing patient engagement, and meeting regulatory and quality reporting requirements.

The analyst is responsible for designing, configuring, supporting, and optimizing Epic Healthy Planet functionality, including care management workflows, patient registries, quality reporting, decision support tools, outreach programs, wellness initiatives, and pharmacist-driven clinical programs. This role ensures Population Health applications are configured in accordance with Epic best practices while supporting organizational goals for preventive care, chronic disease management, risk stratification, and value-based reimbursement.

Working collaboratively with Care Management, Pharmacy, Clinical Operations, Quality, Population Health, Information Services, and operational leadership, the analyst gathers business requirements, translates operational needs into technical solutions, and implements workflow enhancements that improve care coordination, patient engagement, and clinical performance. The role partners with stakeholders to identify opportunities for automation, standardization, and optimization of population health workflows across the organization.

The Epic Population Health Application Analyst serves as the Information Services subject matter expert for Healthy Planet and related Population Health technologies, providing technical consultation, production support, workflow optimization, and end-user guidance. The analyst develops and supports patient registries, reporting workbench reports, dashboards, decision support tools, health maintenance protocols, BestPractice Advisories (BPAs), and integrated outreach solutions that enable proactive patient management and improve quality performance.

This position is responsible for supporting healthcare quality initiatives, including value-based care programs, regulatory reporting, quality measures, and population health analytics. The analyst maintains a comprehensive understanding of healthcare data integration, clinical quality reporting, and interoperability while supporting integrations between Epic and external data sources to enhance patient attribution, risk adjustment, and clinical decision-making.

The analyst participates in application upgrades, integrated testing, workflow redesign, and implementation of new Epic functionality while continuously evaluating system performance, identifying opportunities for improvement, and recommending solutions that advance organizational population health strategies. All work is performed in accordance with HIPAA, organizational privacy policies, information security standards, and applicable regulatory requirements.

• Works with the IS. Team members, vendors and customers to ensure that the application systems are properly maintained and necessary changes are reliably tested, implemented, and documentation is current. Serving as a liaison between end users' workflow needs and implementation staff

• Maintaining regular communication with software representatives, business community, and end users to ensure the system meets the organization's business needs in regards to the project deliverables and timeline

• Developing an understanding of operational needs to set the direction for the organization's workflows by attending site visits and other integrated sessions

• Expert in workflows identifies areas for system optimization and process improvement. Develops solutions and collaborates with the team and management to implement these solutions.

• Works to ensure that user problems and configuration requests are appropriately addressed, and system upgrades are successfully executed with minimal disruption to operations

• Develops and manages effective customer relationships with end-users and other team members to enhance the timeliness and effectiveness of technology solutions.

• Responds promptly to the needs of end-users, and meets Service Level Agreements as defined in conjunction with our customers following issues through to closure.

• Participates in team and leadership meetings to discuss team and ongoing project related activities, issues, change, communications, and updates.

• Typically provides training and guidance to colleagues with less experience, but no formal supervisory responsibility

Required Experience and Professional Skills

Experience

• 5-7 years of experience in Healthcare industry and previous experience in required application discipline

• Current Epic proficiency supporting Epic Healthy Planet or related Population Health applications.

• Experience supporting population health workflows, care management, disease registries, patient outreach, and quality improvement initiatives.

• Experience designing and configuring Epic Healthy Planet functionality, including patient registries, reporting workbench reports, health maintenance, BestPractice Advisories (BPAs), and care management workflows.

• Experience supporting clinical quality reporting, preventive care initiatives, value-based care programs, and population health analytics.

• Experience gathering business requirements and translating operational needs into technical application design.

• Experience supporting production environments, application troubleshooting, workflow analysis, testing, and system optimization.

• Working knowledge of healthcare quality programs, regulatory reporting, and population health best practices.

• Strong analytical, problem-solving, and critical-thinking skills.

• Excellent written and verbal communication skills with the ability to collaborate effectively across clinical, operational, quality, and technical teams.

• Ability to manage multiple priorities while supporting production systems in a fast-paced healthcare environment.
  

Preferred Qualifications

• Epic Healthy Planet Certification.

• Epic certifications in Ambulatory, Care Management, MyChart, or additional clinical applications.

• Three or more years of experience supporting Epic Population Health solutions in an integrated healthcare environment.

• Experience with value-based care programs, Accountable Care Organizations (ACO), MSSP, HEDIS, MIPS, PQRS, or other quality reporting initiatives.

• Experience supporting clinical decision support, patient engagement technologies, and care coordination workflows.

• Experience integrating external clinical and claims data into Epic Healthy Planet.

• Experience participating in Epic upgrades, workflow redesign, optimization initiatives, and integrated testing.

• Knowledge of healthcare analytics, risk stratification, quality dashboards, and population health reporting.
