RN CASE MANAGER

Hospice RN Case Manager – Full-time day shift (8:00 am–4:30 pm) in Murrieta and surrounding areas. Responsibilities Demonstrate knowledge and principles of accepted professional nursing care and apply skills in the home environment. Communicate effectively with clients, physicians, co-workers, supervisors, and the public while following agency communication protocols. Develop, implement, evaluate, and revise client Plans of Care. Observe, evaluate, and report significant changes in a patient’s or family’s physical or psychosocial condition to the physician and the Hospice Interdisciplinary Team; discuss patient problems with a supervisor and assist with plan updates within two days of admission or condition change. Qualifications Current RN license in the State of California. Minimum one year of acute‑care experience. Hospice experience preferred. EMR experience preferred.
