FW Periodic Home Site Supervisor

Sevita

Fort Wayne, IN

Sevita is seeking a FW Periodic Home Site Supervisor to oversee the quality and consistency of home-based supports for individuals in our community programs. In this field-focused role, you will conduct periodic home visits, monitor safety and quality of care, and ensure services align with person-centered plans. You will coach and support direct care staff, provide feedback, and help with training and onboarding. The supervisor documents visit findings, coordinates with families and interdisciplinary teams, and ensures compliance with state regulations and Sevita policies. This position offers the chance to make a meaningful impact while growing your skills through ongoing training in a supportive, mission-driven environment.
