Case Manager Supervisor

General Statement of Duties The Home Base Case Manager Supervisor works under the supervision and direction of their supervisor for the purpose of providing the highest quality of care to children and families. The Home Base Case Manager Supervisor provides in‑home and community‑based family services, including crisis intervention, counseling, skill building, and advocacy. They maintain a 40‑hour flexible work schedule, read and respond to emails, liaise with the Department of Child Services, probation, and community resources. The supervisor assigns referrals as received, reviews and approves progress notes, treatment plans, and monthly reports for compliance, reviews program and policy needs, performs chart audits, and maintains caseloads to meet state audit requirements. They provide direct supervision to up to five staff, covering needs due to illness, injury, or failure of staff to provide direct services. Direct Supervision and Administrative Responsibilities Direct supervision of up to five staff. Assist with conducting weekly staff meetings. Review all case notes of assigned employees. Review and sign all treatment plans. Review all monthly reports. Review hours for time‑sheet reporting. Provide coverage when staff fail to provide services. Assign and track incoming referrals to meet caseload expectations. Initiate and maintain internal and external communication relationships. Provide in‑person supervision with supervisor at least 1 hour per 25 billable face‑to‑face hours. Participate in staff/group consultation meetings. Complete collaborative documentation. Maintain confidentiality concerning clients. Respond promptly (within 1–3 hours) to communications from superiors. Update shared calendar weekly. Establish and maintain positive working relationships with referring agencies. Represent the agency in community meetings and interagency teams as directed. Intervention and Service‑Delivery Responsibilities Schedule and complete client intake within 48 hours of referral. Provide 24‑hour crisis intake, intervention, and consultation seven days a week, primarily in the family's home. Provide ongoing risk assessment and monitoring of family/parental progress. Provide time‑limited services focused on objectives derived from the DCS/Probation case plan or Informal Adjustment. Develop short and long‑term family goals with measurable outcomes aligned with the DCS case plan. Offer family‑centered, child‑focused services including intensive in‑home skill building and after‑care linkage. Provide monthly progress reports, requested supportive documentation, testimony, court appearances, and case conferences/staffing. Submit EON and NCFAS information within 2 weeks of intake. Conduct holistic, strength‑focused assessment processes. Meet face‑to‑face client‑hour guidelines (5/2 ratio). Offer a range of clinical, concrete, and advocacy services tailored to family circumstances. Respond appropriately to potential safety issues. Utilize motivational interventions and cognitive/behavioral strategies. Employ collaborative goal‑setting and teaching approaches. Use theoretically based interventions. Apply effective strategies for engaging, defusing, and confronting clients. Connect families to community resources and informal supports. Continuously evaluate and modify family goals and service plans. Solicit feedback and input from families regarding all aspects of the intervention. Implement termination processes at the end of interventions. Participate in additional program areas as requested by the supervisor. On‑Call and Travel Responsibilities Maintain prompt and regular attendance. Be on call 24 hours a day, seven days a week. Arrange coverage by other case managers or supervisors if unavailable for an extended period. Schedule a flexible workweek to meet families' needs, including evenings, weekends, and occasional holidays. Schedule family meetings at convenient times for families. Provide backup to the supervisor and other home‑based case managers as needed. Travel to client homes and other community locations and transport clients throughout the community. Attend mandatory meetings, training, and continuing education activities. Establish annual goals and review them periodically with the supervisor. Participate in all agency fundraisers. Qualifications Bachelor’s degree in social work, psychology, sociology, or a directly related human service field, and at least two years of social service experience (MSW/MA welcome). Valid driver’s license and automobile insurance. Commitment to the agency’s vision, values, and mission. Knowledge of child abuse and neglect, child and adult development. Knowledge of community resources and teamwork capability. Belief in helping clients change circumstances, not just adapt. Belief in adoption as a viable means to build families. Understanding of issues specific to adoptions such as loss, mismatched expectations, flexibility, loss of familiar surroundings, customs, entitlement, gratification delay, flexible parental roles, and humor. Maintain client confidentiality and confidentiality of all agency records. Ability to work with diverse individuals. Knowledge of crisis intervention, communication, parenting skills, and cognitive and behavioral interventions. Current First Aid and CPR certification. Effective verbal and written communication skills. Familiarity with Microsoft Word, Outlook, and the Internet; ability to write reports and treatment plans. Must live in the designated service area. Knowledge of federal, state, and local rules, regulations, and policies governing home‑based programs. Effective communication with co‑workers, supervisors, clients/families, personnel, and officials from referring agencies. Ability to predict, recognize, and assess potentially dangerous situations and take appropriate action. Ability to cope with the mental and emotional stress of job requirements. Demonstrate personal integrity and sound judgment. Working Conditions Work flexible hours, including evenings, weekends, and holidays. Primary location is the community and client homes. Requires extensive travel to client homes and throughout the community. May be exposed to infectious diseases, substances, contaminated objects, and physically out‑of‑control clients. Weekly contact with staff and non‑staff (client families, referring agencies, community professionals, community agencies/programs).
