PRN Home Health Occupational Therapist (OT) – Flexible Schedule | High Pay Per Visit

We are actively seeking a PRN Occupational Therapist (OT) to support our growing patient base. This is a per diem role designed for flexibility—perfect for clinicians who want to pick up visits based on their availability without being tied to a full-time schedule.

Responsibilities:

• Perform initial and ongoing patient evaluations
• Develop and implement individualized plans of care
• Provide therapy focused on activities of daily living (ADLs) and functional independence
• Complete OASIS assessments as required
• Educate patients and caregivers on treatment plans
• Maintain timely and accurate documentation
• Communicate patient status and changes with the care team

Qualifications:

• Active Occupational Therapist license in the state of employment
• Minimum of one year clinical experience (home health preferred)
• Strong assessment and documentation skills
• Reliable transportation and valid driver’s license
• Ability to work independently in the field

Compensation:

• $50 – $80 per hour equivalent, based on visit type and productivity as well as location
• Mileage reimbursement available
