Registered Nurse (RN) - Homecare & Hospice

Location: North Conway, NH

Employment Type: Full-Time / Part-Time | 24-40 hours/week

Setting: In-Home Care / Home Health & Hospice

Compensation: $37.02-$50.09/hour, based on education and experience

Key Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Provide skilled nursing care, including wound care, medication administration, symptom management, and patient education.
• Coordinate care with physicians and other healthcare professionals.
• Support patients and families with education and emotional guidance.
• Maintain accurate documentation in the EMR.
• Help promote quality outcomes and reduce avoidable hospitalizations.

Schedule

• Day Shift: 8:00 AM - 4:30 PM
• Hours: 24-40 hours/week
• Weekend Call: Approximately once per month
• Shift Differential: Available for applicable on-call, weekend, and night coverage.
• Travel to patients' homes is required.

Qualifications

• Active RN license in New Hampshire and Maine
• New graduates are welcome
• Home care, long-term care, and/or hospice experience preferred
• Strong assessment and critical-thinking skills
• Ability to work independently in a home-based environment
• Comfortable using EMR/mobile documentation systems
• Valid driver's license and reliable transportation required

Patient Population

• Various age groups, with mostly adult patients
• Approximately 5 patient visits per day, depending on patient needs and scheduling
