Pharmacist
The Pharmacist is responsible for providing pharmacy consulting services to patients regarding the effective usage of medications, awareness with drug interactions and offering preventive healthcare services. Responsible for ensuring the proper dispensing, review, and verification of prescribed medications within regulatory guidelines, company policies and procedures. Supports the efficient workflow of the pharmacy and assists the pharmacist in charge in identifying ways to optimize pharmacy financials, inventory management, and the patient experience.
Essential Job Responsibilities:
- Patient Experience: Engages patients by greeting them and offering assistance with products and services. Resolves patients issues in timely manner and answers questions to ensure a positive patient experience.
- Operations: Counsels patients and answers their questions regarding usage of medicine, side effects, interactions, contraindications, over-the-counter products, and collaborates/refers to medical provider as needed to ensure medication is taken correctly, health needs are addressed, and patient is satisfied with service levels received.
- People and Performance Management: Assists the pharmacist-in-charge with team member training and hiring. Reinforces the direction of the pharmacist-in-charge in team member performance by coaching and providing constructive feedback, monitoring patient service, and fostering team member development.
- Training and Personal Development: Maintains a current knowledge and skills related to pharmacy and healthcare by reading pharmacy related journals, company publications and communications. Remains current on all CE requirements required by state.
- Communication: Supports the pharmacist-in-charge by communicating relevant health and wellness services or strategy information to team members and/or medical providers.
- Quality Assurance/Compliance: Completes all required training. Participate in regular case staffing to discuss procedures and quality assurance with the medical team.
- Safety: Ensures staff maintain agency guidelines relating to safety, outreach and confidentiality. Ensures proper hand washing according to Centers for Disease Control and Prevention guidelines. Documents patient's medications correctly, makes sure each patient knows which medicines to take when they are at home and encourages each patient to bring their up-to-date list of medicines every time they visit the doctor. Ensures each new client receives screening for their risk for suicide. Understands and appropriately acts upon assigned role in Emergency Code System. Understands and performs assigned role in Agency Continuity of Operations Plan (COOP) Ensures outposted staff understand the safety requirements of the location in which they are posted.
Job Knowledge and Skills: Bilingual (English Spanish or English-Creole) is preferred. Computer knowledge. Pioneer RX Software Knowledge a plus. Knowledge of the disease concept of chemical dependency, chemical dependency treatment, and group dynamics. Microsoft Outlook, Word, and Excel. Excellent organizational, communication, problem solving, and teamwork skills are required to provide adequate treatment to patients, make referrals to appropriate specialists for diagnostic purposes and prioritize patient care issues and standards of care. Able to work with a multicultural and diverse population is required. Sensitivity and knowledge of the LGBT community is highly recommended.
Physical Requirements: This work requires the following physical activities: constant standing/sitting, talking in person, talking on the phone, hearing/visual acuity and exposure to blood borne pathogens. Frequent walking, standing and exposure to hazardous materials. Occasional bending, stretching/reaching and driving may be required. Work is performed in office and medical settings.
