Patient Intake Coordinator Key Responsibilities: Serve as the first point of contact for new patient referrals.Communicate with patients, caregivers, and insurance representatives to complete the intake process.Verify eligibility, collect required documentation, and ensure all pre-enrollment steps are completed accurately.Coordinate with nursing, compliance, and staffing departments to ensure a smooth start of care.Assist with unapproved patient assessments.Maintain detailed records in internal systems and ensure compliance with all DOH and agency guidelines.
