Role Overview: A registered nurse in home health provides personalized, one-on-one nursing care in patients' homes, focusing on recovery, safety, and independence. Key Responsibilities Conduct comprehensive in-home assessments to develop patient-centered care plans. Deliver skilled interventions such as wound care, infusions, medication management, and post-operative care. Educate and empower patients and caregivers to manage symptoms and treatments confidently. Monitor patient progress, identify early changes in condition, and communicate with the care team to adjust plans. Coordinate care across providers, ensuring continuity and high-quality outcomes. Complete accurate documentation supporting compliance and measurable results. Qualifications & Skills Active RN license in the practice state. Minimum one year of clinical nursing experience. Patient-focused, organized, and motivated by delivering quality care. Reliable transportation, valid driver’s license, and CPR certification (or ability to obtain).
