Social Services Director

At Richboro Rehabilitation & Nursing Center, we are seeking a compassionate and experienced Social Services Director to join our interdisciplinary team.

Richboro is dedicated to providing top-quality skilled nursing care for short-term rehabilitation and long-term care, and we are looking for a Social Services Director who shares our commitment to excellence.

Job Summary:

You will educate staff on the role of social services, lead the development of written care plans, and hold meetings with patients/residents, families, and significant others to provide support and information.

You will also perform discharge planning activities, establish relationships with community-based agencies, and maintain contact and referral flow.

Responsibilities:

Collaborate with the interdisciplinary team to promote and protect resident rights and the well-being of all patients/residents
Educate staff on the role of social services and participate in staff development activities
Lead the development of written care plans for each resident's social service needs
Hold meetings with patients/residents, families, and significant others to provide support and information
Perform patient/resident discharge planning activities, including family/patient education, nursing center documentation, and referrals to appropriate agencies and suppliers
Establish relationships and maintain contact and referral flow with community-based agencies and services

Qualifications:

Bachelor's degree from an accredited school of Social Work or related field
Licensed in Social Work for the state of PA
