Client Care Coordinator

Full Time Patient Care Coordinator Helping Hand Nurse is a home health care agency servicing the greater Philadelphia area and surrounding counties. We are currently looking for a full time patient care coordinator. The coordinator will be responsible for: Coordinating patient care with field staff, doctors, and organizations Conducting orientation/training for new caregivers Ensuring compliance with caregiver clock-in and clock-out requirements Requesting authorizations Comfortable visiting patients in their homes when needed for care management Developing relationships with different service coordinators Increasing satisfaction and caregiver/patient retention Creating patient schedules and assigning staff to appropriate schedules promptly Helping Hand Nurse offers unique benefits that includes: Flexible schedule/work life balance Competitive salary Medical benefits PTO Be a part of an award-winning team Dental insurance Health insurance Life insurance Paid time off Paid training Vision insurance Qualifications include: At least one year of home care experience is preferred but training is provided Have the ability to present a professional appearance and demeanor Be patient and congenial on the phone Strong work ethic Demonstrate excellent oral and written communication skills and the ability to listen effectively Have the ability to work independently, maintain confidentiality of information and meet deadlines Demonstrate effective interpersonal skills as well as sound judgment and good decision-making skills Bilingual (Spanish) preferred but not required Job type: Full-time Schedule: 8 hour shift Monday to Friday Work setting: In-person Office Work location: In person
