Position Summary

We are seeking an experienced Licensed Practical Nurse (LPN) to provide nursing care to residents in a Long-Term Care (LTC) / nursing home setting .

The LPN will be responsible for delivering safe and compassionate resident care, monitoring resident conditions, administering medications and treatments as assigned, and collaborating with the healthcare team to support quality patient outcomes.

The position requires flexibility to work a 3×12-hour day-shift schedule , including every other weekend.

Key Responsibilities

• Provide nursing care to residents in accordance with established care plans and facility policies.
• Monitor residents' health status and report changes in condition to the appropriate healthcare provider.
• Administer medications and treatments within the scope of LPN practice.
• Document resident care, assessments, medications, and other clinical information accurately.
• Assist with activities of daily living and provide appropriate nursing support.
• Communicate effectively with residents, families, physicians, and other members of the healthcare team.
• Follow infection-control, safety, and regulatory requirements.
• Participate in maintaining a safe and supportive environment for residents.
• Perform other duties appropriate to the LPN role as assigned.

Required Qualifications

• Active LPN license required.
• Previous Long-Term Care / Nursing Home experience preferred.
• Ability to work 36 hours per week on the required rotating weekend schedule.
• Strong clinical, communication, and documentation skills.
• Ability to work effectively as part of a multidisciplinary healthcare team.
