Pay Rate: $33 - $36/hr

Position Overview

We are seeking an experienced LVN/LPN to provide nursing care, medication administration, and clinical support to residents in a long-term care setting.

Required Qualifications

• Active New York State LVN/LPN license, verified at submission.

• Minimum 1 year of Long-Term Care (LTC)/Skilled Nursing experience.

• Current BLS certification.

• Experience caring for Alzheimer’s and dementia patients preferred/required for the patient population.

• Successful completion of medication administration (med pass) test and clinical observation.

• Strong medication administration and clinical assessment skills.

• Strong attendance, punctuality, and reliability.

• First-time travelers accepted.

• Flexibility with scheduling needs.

Responsibilities

• Administer medications and treatments safely according to physician orders.

• Perform medication passes and monitor residents for effectiveness and adverse reactions.

• Provide nursing care according to facility policies and established care plans.

• Monitor resident conditions and report significant changes to appropriate healthcare providers.

• Provide care to residents with Alzheimer’s, dementia, and other long-term care needs.

• Complete accurate and timely clinical documentation.

• Supervise CNAs and coordinate resident care activities.

• Communicate effectively with residents, families, CNAs, nurses, providers, and interdisciplinary team members.

• Maintain resident safety and follow infection-control and facility policies.

• Support a positive, professional, and respectful environment for residents.
