Drive better patient outcomes by proactively implementing and monitoring personalized care plans. Collaborate across disciplines to create a seamless, well-coordinated care experience for every patient. Support the broader medical team by executing physician and nursing orders with precision and attention to detail. Champion the patient’s transition home by assessing individual needs and orchestrating discharge planning in partnership with the full care team. What Minimum Requirements You’ll Need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. One or more of the following required: Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date. Education Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
