Service Coordinator - Children's Specialized Hospital Location: 150 New Providence Rd, Mountainside, NJ 07092 Department: Case Management Pay Range: $54,080.00 - $75,920.00 per year Status: Full‑Time, Day Shift Job Overview The Service Coordinator facilitates coordination of services for developmentally delayed children who qualify for Early Intervention, assists child and family in the development of the Individualized Family Service Plan, provides supportive counseling and crisis intervention, and advocates for children on state and local levels. Qualifications Bachelor's degree or higher in a related health, human service, psychology, or education field. At least one year of professional experience working with children and families in counseling, supporting, or case management roles. Ability to work independently with a demanding client caseload. Ability to provide supportive counseling and crisis intervention. Exhibits empathy and cultural ethnic sensitivity to a diverse client population. Effective problem‑solving skills. Preferred: Master's degree. Social Work License. Experience with individuals birth to five years of age and their families. Scheduling Requirements Full‑time, 40 hours per week. Essential Functions Establish working relationships with developmentally delayed children and families. Coordinate scheduling of assessments per Early Intervention and DOH guidelines. Assess children's service needs across social, emotional, developmental, familial, medical, cultural, economic, and educational areas. Develop and review Individualized Family Services Plans (IFSP) for eligible Part C children. Ensure appropriate notification is provided. Facilitate coordination of services through linkage to providers and EI programs. Promote and facilitate communication among service providers. Advocate for appropriate services. Provide supportive counseling and crisis intervention. Assist families with transition planning to preschool, special education, or other services at age three. Manage electronic files, including data entry into EIMS and DOH database. Assume unit functions in the absence of the Unit Coordinator. Participate in development, evaluation, and revision of unit policies and procedures. Other Duties This description is not a comprehensive list of all duties and responsibilities.
