License Practical Nurse (LPN)

We are seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our Wellness team on a part‑time or per diem basis . The LPN will provide hands‑on resident care, administer medications, maintain accurate documentation, and collaborate with the Registered Nurse and care team to support residents’ health, safety, and well‑being. Responsibilities Provide resident care in accordance with individualized service plans. Assist residents with activities of daily living as needed. Administer prescribed medications and accurately document all services provided. Monitor residents for changes in health, behavior, or functional status. Promptly communicate resident concerns or changes in condition to the Registered Nurse and appropriate healthcare providers. Assist with developing and updating resident service plans and clinical documentation. Respond to resident emergencies, provide first aid, and arrange appropriate medical follow‑up. Maintain accurate records in communication logs, service plans, and daily care documentation. Provide compassionate one‑on‑one support to residents as directed. Help maintain a safe, comfortable, and respectful environment. Support the Registered Nurse with staff training, assignments, and other clinical responsibilities as needed. Follow all community policies, procedures, and applicable healthcare regulations. Promptly report resident incidents and work‑related injuries. Qualifications Current and active Licensed Practical Nurse (LPN) license in the State of New Jersey. Successful completion of an approved practical nursing program and required licensing examination. Ability to work collaboratively in a fast‑paced healthcare environment. Strong communication, organizational, and documentation skills. Compassionate bedside manner and excellent attention to detail. Experience in assisted living, senior living, or long‑term care is preferred. Shift 3:00pm-11:00pm 11:00pm-7:00am Every other weekend $32 - $32 an hour Why Join Spring Oak?
