Registered Nurse Case Management Hospital

Job Summary:

Provide specialized case management nursing care to patients within a hospital setting, coordinating services and resources to ensure quality patient outcomes and efficient discharge planning.

Responsibilities:

• 
  Assess patients clinical needs and develop individualized care plans.
  
  
• 
  Collaborate with healthcare team members to coordinate patient care.
  
  
• 
  Educate patients and families regarding treatment plans, medications, and post-discharge instructions.
  
  
• 
  Maintain accurate documentation of patient progress and interventions using electronic health records.
  
  
• 
  Facilitate communication between patients, families, and healthcare providers.
  
  
• 
  Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
  
  

Qualifications:

• 
  Current Registered Nurse (RN) license in the state of North Carolina or applicable state.
  
  
• 
  Minimum of 2 years of clinical nursing experience, preferably in a hospital or case management setting.
  
  
• 
  Strong communication and interpersonal skills to build trust with patients, families, and team members.
  
  
• 
  Ability to work collaboratively within a multidisciplinary team.
  
  
• 
  Proficiency in electronic documentation systems such as CERNER, NATE, and ENSOCARE.
  
  
• 
  Demonstrated ability to prioritize tasks and manage shifting attention efficiently.
