Position Summary The Registered Nurse (RN) will play a pivotal role in guiding patients and their families through the healthcare system. Their primary focus is to provide compassionate, patient-centered care while acting as an advocate for those they serve. The RN will be a vital member of the multidisciplinary healthcare team, dedicated to delivering high-quality nursing services across adult primary care, women’s health, and pediatrics. Core Values Inclusion Nurture Service Purposeful Integrity Resilient Engaged Essential Qualifications Graduation from an accredited nursing program Licensed with the AZ State Board of Nursing (in good standing) Two (2) to five (5) years of experience in a clinical setting Experience as a Registered Nurse in OB/GYN and Pediatrics Demonstrated application of Nursing Planning (Assessment, Nursing Diagnosis, Goals/Outcomes, Implementation, and Evaluation) Demonstrated understanding of clinical methods and techniques used in record-keeping systems Demonstrated ability in implementing the principles and practices of supervision Good working knowledge of Microsoft Office applications (Outlook, Word, Excel) Ability to write analytic reports integrating multiple data elements and perspectives Familiarity with Adelante healthcare mission, vision, and core values and use in patient care and collaboration Ability to maintain confidentiality, privacy, and security of protected information Excellent verbal and written communication skills Travel to all health centers is required as needed Advanced computer efficiency in the Microsoft Office suite (Word, Excel, PowerPoint, Outlook) and experience in the Patient Management System Meet the organization's immunization requirement Valid Arizona driver’s license and active automotive insurance Certification to perform CPR and AED following AHA and Red Cross guidelines Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety Participation in Community Events as required Position Responsibilities Patient Care Responsibilities Perform triage phone calls and walk‑in patient assessments using approved procedures for adult primary care, women’s health, and pediatric populations. Assist providers with wound care, urinary catheterization, suture/staple removal, wart removal, lacerations, skin biopsy, abscess incision and drainage, toenail and skin tag removal, LEEP, colposcopy, and IUD insertion. Perform EKG, peak flow, spirometry, albuterol and oxygen treatments and document results in the electronic health record. Clinical Collaboration Collaborate with the clinical team to review and act on tasks/PTAs sent by providers. Assist with point‑of‑care testing (HbA1c, UA, HCGs, hemoglobin, fecal occult blood test, lead screening, PKU, HIV, strep) with the FM/IM interdisciplinary team. Support health‑center staff with vaccine and medication administration, including NG‑G‑tube (Rota vaccine). Perform PPD placement, flu and COVID testing. Support referral process per AHC standing orders and procedures. Develop self‑management goals using SMART framework. Troubleshoot and problem‑solve using critical thinking during complex situations. Review and contact patients regarding normal and abnormal lab and diagnostic results. Quality Improvement and Coordination Collaborate with interdisciplinary team to improve Uniform Data System (UDS) quality metrics. Improve care delivery integration and close HEDIS care gaps with the integrated care team. Coordinate outreach to high‑risk patients, hospital discharge and admissions, collaborating with health plans, hospitals, and skilled nursing facilities for transitional care. Assist with tasks/PTA messages about insulin titration for diabetics, including education and follow‑ups. Identify barriers to care and social determinants of health by ensuring PRAPARE survey is collected. Support high‑risk prior authorizations, durable medical equipment (DME), and short‑term disability as needed. Ensure compliance with Joint Commission, HRSA, and AHC standards. Leadership and Accountability Provide leadership for collaboration and coordination of services, defining appropriate skill mix and delegating tasks among licensed and unlicensed staff. RNs are fully accountable in ambulatory care settings for all nursing services and associated patient outcomes under clinical providers. Patient Education and Reporting Provide education and reporting on STIs as needed for public health. Build relationships with external healthcare organizations and facilities to find additional resources for patient care. Administrative Responsibilities Obtain medical records, labs, and diagnostic results as needed. Align role and responsibilities with Adelante Healthcare's mission, vision, core values, policies, and procedures. Ensure proper, complete documentation using approved templates in the electronic health record. Conduct medication reviews, reconciliation, and refills as needed. Evaluate clinical resources and support development of new tools, forms, and procedures. Participate in team meetings as needed. Skills and Competencies Apply critical thinking and organizational skills in all tasks. Enhance patient safety and quality of care per Board of Nursing scope and organizational standards. Family Medicine (FM)/Internal Medicine (IM) RN Collaborate on Medicare Wellness Visits by conducting assessments, completing surveys, reviewing charts, and documenting appropriately. Provide direct nursing care to adult patients (care coordination, education, triage, health assessment, medication and vaccination administration, record management, quality improvement). Provide education to patients and families on health management, disease prevention, and wellness promotion. Women’s Health RN Perform and interpret NSTs, documenting results and communicating findings. Assist with pap smears, breast exams, contraceptive education. Provide direct nursing care for women’s health patients (care coordination, education, triage, medication, vaccination, health assessment, record management, quality improvement). Pediatrics RN Provide direct nursing care to pediatric patients (coordination, education, triage, health assessment, record management, medication, vaccination, quality improvement). Educate parents and caregivers on child health, disease prevention, nutrition, and wellness. Assist providers with procedures such as circumcision. Quality Methodologies Maintain knowledge of PDSA, DMAIC, ADKAR, SMART models. Collaborate in developing continuous quality improvement plans. Other projects as requested by Nursing Manager, Quality Improvement Manager, or Director of Quality. Non‑Essential Skills and Experience Electronic Health Record experience. Bilingual (Spanish/English). Ability to read/write in more than one language. Bachelor’s degree preferred. Other non‑essential options. Additional Duties and Responsibilities Other duties as requested or assigned. Patient Centered Medical Home (PCMH) and Integrated Care Team (ICT) Promote and participate in interdisciplinary communication and collaboration to enhance delivery of quality care across the health system. Adherence to Compliance and Code of Conduct Comply with Adelante Healthcare’s written standards, compliance program, code of conduct, policies, and procedures; report any conduct potentially violating legal or compliance requirements; compliance considered part of performance evaluation. Physical Demands and Work Environment May sit for long periods, stand, walk, use hands, reach with arms, climb stairs, balance; stoop, kneel, crouch, or crawl as required. Manage stress; talk or hear; smell. Lift up to 25 pounds frequently and up to 50 pounds with assistance. Vision: close, distance, color, peripheral, depth perception, ability to adjust focus.
