Registered Nurse - Case Management - Hospital

$2611.20/week –

Job Summary:

Seeking an experienced Case Management RN to coordinate patient discharges, provide post-surgery education, and facilitate navigation within the hospital setting. The ideal candidate will possess strong clinical background and expertise in orthopedic cases.

Responsibilities:

• Collaborate with multidisciplinary teams to develop and implement discharge plans.

• Educate patients on post-operative care and ensure understanding of treatment plans.

• Utilize Parachute and Careport software for case documentation and management.

• Coordinate utilization review and manage patient navigation to ensure efficient care delivery.

• Maintain accurate records and ensure compliance with regulatory standards.

• Review patient cases for discharge readiness and communicate effectively with healthcare providers.

Qualifications:

• Hold a valid NYS RN license and BLS certification.

• Possess 3-5 years of clinical experience in medical-surgical, ICU, or ER settings.

• Experience with orthopedic case management, including discharge planning and patient education.

• Familiarity with Parachute and Careport software applications.

• Certified in HC-PRI and PRI screening; CCM certification preferred.

• Demonstrate strong communication and organizational skills.
