Occupational Therapist (OT) - Home Care

Relocation bonus available for >50mi The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages to resume or adjust the activities of daily living. Collaborates with the health care team and modifies plan of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending upon census and program need. Qualifications Certifications: Basic Life Support (BLS) within 30 days - American Heart Association; Driver’s License – Illinois Secretary of State; Licensed Occupational Therapist (OT) – Illinois Department of Financial and Professional Regulation; Proof of Auto Insurance. Education: Bachelor’s Degree in Occupational Therapy (Required). Work experience: Minimum of X years in home care occupational therapy. Responsibilities Reviews plan of care and orders with primary physician, documents read‑back, and ensures timely transmission per policy. Educates patients and family members, overcoming barriers and matching learning capabilities. Documents visit details thoroughly to enable continuity of care. Updates goals promptly and closes achieved goals. Initiates case conferencing and documents interactions with other disciplines, MDs, care coordinators, insurers, etc. Documents care plans and patient/family assessments demonstrating therapy knowledge, assessment skills and critical thinking. Anticipates and orders supplies to prevent shortages between visits. Educates and documents discharge planning, coordinating with physicians, insurers, outpatient visits and community resources. Completes audits and assigned tasks within stipulated time frames. Evaluates patient’s level of function within regulatory requirements, applying diagnostic and prognostic procedures. Guides patient in therapeutic, creative and self‑care activities to improve function. Observes, records and reports patient reactions to treatments and any condition changes. Instructs other health team personnel (OTAs, HCAs, family members) as appropriate. Communicates OTA assignments with scheduler, prepares written instructions and supervises OTA at patient homes. Maintains understanding of home‑care rules and regulations and their influence on case mix index. Completes and documents OTA supervisory visits (at least every 6 visits); completes HCA supervisory visits every 14 days if therapy only. Completes comprehensive OASIS assessment, assesses homebound status and performs occupational therapy evaluation and plan of care in patient’s home. Schedules appointments and manages daily schedule. Assesses home safety and recommends equipment to improve safety. Educates family/caregivers on home exercise program.
