Cincinnati, United States | Posted on 09/24/2026 Director of Wound Care - Nurse Practitioner Job Summary We are seeking an experienced and clinically strong Directorof Wound Care to lead our wound care service line across partneredskilled nursing, assisted living, and long-term care facilities. This role isresponsible for both clinical oversight and operational leadership,including advanced wound procedures, team mentorship, quality improvement, andservice line growth. The Director of Wound Care will maintain clinical competencyin advanced wound care procedures, provide direct oversight of cliniciansperforming these procedures, and own the performance metrics for the serviceline. This position combines hands-on clinical work with strategic leadership,requiring regular travel to partner facilities and collaboration withcross-functional teams. Key Responsibilities Leadthe wound care service line, including clinical oversight, operationalperformance, and growth strategy. Maintaindirect clinical competency in advanced wound procedures, includingdebridement and skin graft application. Overseeand mentor wound care clinicians performing sharp, mechanical, andenzymatic debridement, as well as skin substitute/graft procedures. Establishand maintain clinical protocols for wound assessment, staging, treatmentplanning, and escalation based on evidence-based guidelines. Ensureappropriate diagnostic workup is ordered, including labs, vascularstudies, imaging, and cultures as clinically indicated. Supportclinicians in identifying the underlying causes of wounds and factors thatmay impair healing. Monitorand improve clinical outcomes, documentation quality, procedure volume,and financial performance. Ownand review the KPI dashboard for the wound care service line. Ensureaccurate documentation, coding, and billing for wound care procedures. Collaboratewith partner facilities to improve wound outcomes and reduce avoidablehospitalizations and emergency department visits. Buildstrong relationships with skilled nursing, assisted living, and long-termcare partners. Supportteam development, retention, and certification compliance. Travelregularly to partner facilities to provide hands-on oversight, mentorship,and program support. Required Qualifications Active,unrestricted Nurse Practitioner (NP) license in Kentucky,West Virginia, or Ohio; multi-state licensure strongly preferred. Currentboard certification in wound care (such as WCC, CWCN, CWS, orequivalent recognized certification). Minimum 5years of clinical wound care experience, including hands-onperformance of debridement and skin graft procedures. Minimum 2–3years of clinical leadership, supervisory, or program managementexperience overseeing other clinicians. Experienceproviding wound care in skilled nursing, assisted living, orlong-term care settings. Workingknowledge of CMS documentation and billing requirements fordebridement, skin substitute/graft application, and related procedures. Strongclinical judgment, leadership, communication, and organizational skills. Abilityto travel regularly to partner facilities across multiple states.
