Palliative Care Nurse Practitioner

Nurse Practitioner | Palliative Medicine Location: Escondido, CA Employer: Lancesoft Inc Pay: Competitive weekly pay (inquire for details) Start Date: ASAP About the Position Seeking an experienced Locum Hospice & Palliative Care Nurse Practitioner to provide compassionate, home based care for patients with serious or life limiting illnesses in the Escondido area. • • • Assignment starts September 28, 2026 • • • 13 week assignment • • • Approximately 45–50 patient visits per month • • • Home based patient care with local travel • • • Work closely with the interdisciplinary hospice team • • • Serve as a clinical resource for home health aides • • • My Unity EMR • • • Mileage reimbursed at $0.70 per mile • • • Conduct face to face hospice & palliative care visits in patients' homes • • • Perform comprehensive patient assessments • • • Develop & manage individualized care plans • • • Assess & manage symptoms associated with serious illness • • • Conduct goals of care discussions • • • Support advance care planning, including POLST & advance directives • • • Provide patient & family education • • • Coordinate care with the interdisciplinary team & community physicians • • • Participate in IDT meetings & quality initiatives • • • Document patient encounters accurately in the EMR • • • Collaborate with the Hospice & Palliative Care Medical Director • • • Provide clinical guidance & support to home health aides Requirements • • • Nurse Practitioner with hospice or palliative care experience • • • Active California NP & RN licenses • • • DEA required • • • NPI required • • • MSN required • • • AANP or ANCC certification required • • • BLS certification required • • • 3 years of hospice or palliative care experience required • • • ACHPN certification preferred at hire & required within one year • • • Valid California driver's license & automobile insurance required • • • Spanish bilingual skills are a plus • • • Compassionate, independent & team oriented approach • • • Monday through Friday • • • 8:00 AM–5:00 PM • • • One Sunday per month required • • • Weekday off provided when working the Sunday rotation • • • On call coverage may be available/required as needed • • • Participation in 24/7 patient coverage when needed • • • Flexible home visit schedule within assigned coverageaa415a4b-8b21-40fc-a65c-70d2b25ca29a
