Details Home health setting serving a geriatric patient population Average caseload of 5–7 patients per day (to be confirmed during interview) Territory covers up to a 60-mile radius from the hub/office Day shift schedule, 5x8-hour shifts (8:00–17:00) On-call required every 3rd to 4th weekend EMR: HCHB experience preferred but not required OASIS experience preferred but not required iPad provided for documentation Requirements Minimum 1 year of recent home health PT experience required Current BLS certification required Active PT license required if applicable OASIS experience preferred HCHB EMR experience preferred Must be flexible with territory coverage up to a 60-mile radius from the hub/office Additional Information Timely documentation and clear communication are expected in this role Mileage reimbursed at the IRS standard rate Scrubs required in any color; jeans are not permitted Personal cell phone is not required for this role Local candidates are accepted and eligible for the travel pay rate First-time travelers are welcome to apply All time‑off and scheduling requests must be submitted at the time of initial consideration Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax‑free stipend amount to be determined. #J-18808-Ljbffr American Traveler
