Case Manager

We are seeking a dedicated Case Manager to work at the County Line office and provide visiting nurse services to group homes. This position requires a reliable vehicle as mileage reimbursement is provided. Enjoy a balanced work-life with no weekends or holidays.

This position follows a full-time schedule from 09:00 AM to 05:30 PM, Monday through Friday.
