RN Clinical Liaison

Role The Admission RN assesses patients in their place of residency or in hospitals and initiates care into the services of Chapters Health System. The RN ensures a seamless transition to Care Navigation and serves as an ambassador of the organization. Qualifications Current RN license in the state where the employee will be working. Minimum of one (1) year of nursing experience; hospice or hospital experience preferred. Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system. Strong clinical assessment skills; preferred background in emergency department or ICU. Ability to communicate effectively in English (verbal and written); bilingual in Spanish preferred. Mobile driver with a valid driver’s license and automobile insurance per company policy. Ability to manage multiple priorities and handle the emotional stress of the workload. Physical capability to lift patients or equipment, bend, push/pull, kneel, and perform other basic physical tasks. Strong goal/outcome orientation with exceptional time‑management skills. Excellent customer service focus. Active BLS certification for healthcare professionals from the American Heart Association or Red Cross. Competencies Satisfactorily complete competency requirements for this position. General Responsibilities Represent the company professionally at all times through care delivered or services provided to clients. Comply with all state, federal, and local regulations, including fraud and abuse prevention. Adhere to company policies, procedures, and standard practices. Observe the company’s health, safety, and security practices. Maintain patient confidentiality and handle sensitive situations appropriately. Use resources in a fiscally responsible manner. Promote the company through community and professional organizations. Participate proactively in improving performance at the organizational, departmental, and individual levels. Advance electronic media skills and share expertise with co‑workers. Participate in quality assessment and performance improvement activities appropriate to the position. Job Responsibilities Complete clinical admissions for all company services. Explain services to potential patients and families and address questions to alleviate fears and physical limitations. Identify admission objections and provide appropriate responses. Educate patients and families on admission requirements and address immediate needs. Complete informed consents and initial assessment in accordance with regulations and company policy. Provide information to physicians and initiate a plan of care to address patient’s immediate needs. Initiate skilled nursing interventions to prevent complications, alleviate symptoms, and maximize physical and emotional comfort. Obtain physician orders and document per company policy. Act as the company representative at assigned facilities and facilitate referrals to all service lines. Work closely with referring hospitals, physicians, facilities, patients, families, and the general public. Communicate frequently with Care Navigation staff to ensure timely follow‑up and inform the team upon assignment completion. Use SBAR for all clinical communication. Discuss potential needs with after‑hours staff. Obtain consents for services and arrange safe discharge efficiently for referred patients. Obtain medical record information for referral. Develop strong relationships with case managers, physicians, and other professionals at facilities. Work with Professional Relations Representatives (PRRs) to provide education on all available services. Provide care for GIP patients in a hospital, including daily visits, professional management, documentation, and efficient discharge. During emergencies (e.g., hurricanes), the triage RN may be required to report to a designated location and may need to work ahead of schedule or stay overnight. Perform other duties as assigned. Physical Demands Use hands, fingers, and arms to reach, lift, push, pull, bend, balance, and kneel. Stand, sit, walk, and perform basic tasks such as picking up items from the floor. Occasionally push, pull, stoop, crouch, or climb stairs. Requiring sight, hearing, speaking, and walking. Compensation Pay Range: $67,976.53 – $106,213.33 Drug Testing & Background Screening All applicants will be required to consent to drug and/or alcohol testing after a conditional offer of employment and will participate in ongoing compliance with the Drug‑Free Workplace Policy. Employees performing services for Florida affiliates will undergo background screening through the Florida Care Provider Background Screening Clearinghouse before and after the conditional offer. #J-18808-Ljbffr Chapters Health System
