Job Details: Job Location: Priest Drive - Tempe, AZ 85281. Position Type: Full Time. Education Level: High School Diploma/GED. Salary Range: $17.87 - $21.37 hourly. Travel Percentage: Up to 80%. Job Shift: Day Shift. Job Category: Behavioral Health/Social Work. Full‑time position (40 hours per week) at the Priest Drive Recovery Health Center, near Loop 202/Priest. Up to 80% time spent in the field with 20% in the office. Schedule: Monday‑Friday; two shifts available: 7:30am‑4:30pm or 8:30am‑5:30pm. No overnight or weekend work. Compensation: $17.87‑$21.37+$3 per hour differential pay (based on experience and education). Includes a full benefits package with 401(k). Generous paid time off (4+weeks in first year). Additional language differential pay (1$ per hour for Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili or ASL). Duties Provides direct, face‑to‑face community‑based services to assist clients in obtaining and maintaining housing. Conducts screenings, intakes, and assessments to evaluate clients’ presenting problems and develop treatment goals. Provides outreach to engage clients in PSH services and orients them to the H20 program. Provides transportation to clients when medically necessary (in a personal or Terros Health vehicle). Ensures collaboration by communicating appropriate clinical information with families, medical providers, community resources, and other supports. Assists clients with connecting to physical health services, including primary‑care appointments. Provides excellent customer service to clients, providers, state agencies, and stakeholders such as probation officers and advocates. Client Support Stages Pre‑tenancy (3‑6months): Help clients apply for housing vouchers and identify housing options. Tenancy (3‑18months): Sign leases and provide ongoing support to maintain housing, including independent‑living skills and substance‑use services. Caseload: up to 15 members/patients. Required to contact each member at least 4 times per week (phone calls included). Qualifications High School Diploma/GED with a minimum of 1.5years of behavioral‑health experience, or Associate’s degree in medical or behavioral health, or bachelor’s degree (not in behavioral health) with 1year of experience, or bachelor’s degree or higher in behavioral health. Strong customer‑service and communication skills. Ability to work independently, with flexibility and within prescribed deadlines. Intermediate skills in Microsoft Word, Excel, and Outlook; experience with PowerPoint and NextGen electronic medical record is preferred. Valid Arizona driver’s license, age 21‑plus, with a minimum of 4years driving experience and compliance with the company driving policy. Valid Arizona Fingerprint Clearance card (Level1) or ability to obtain one within 7working days of employment. Pass background check, TB test, and other pre‑employment screenings.
