TOC Clinical Pharmacist

Clinical Pharmacist Responsibilities The TOC Clinical Pharmacist shall support the mission of the hospital by aiming to reduce hospital readmissions through patient discharge follow‑up. Perform clinical duties and engage in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourage provider follow‑up and continuity of care following hospital discharge.
