RN Clinical Referral Liaison — Patient Care Navigator

Northwestern Medicine is seeking a Registered Nurse Clinical Referral Liaison for a full-time day shift at the hospital campus. You will clinically assess referred patients, develop a rounding schedule, and coordinate with physicians, discharge planners, and admissions staff to ensure a smooth transition to rehabilitation services.
