Case Manager Prescription Assistance
As part of the Access to Care team, you work in close communication with other case managers at Stephens Hospital and across the MaineHealth system to address Social Drivers of Health and provide healthcare navigation and financial resource strain for our patients. Team members utilize a variety of tools, systems, and resources to match patient needs with available resources.

Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: Associate or bachelor's degree in social services, pharmacy, or healthcare field preferred.
- License/Certifications: N/A
- Experience: Two years of experience in social/human services, pharmacy, healthcare management, or related field required. Community outreach experience and networking required. Experience in public speaking required.
