Responsibilities:

• Provide direct patient care to residents, including assisting with daily living activities, administering medications, and monitoring vital signs

• Assist with maintaining a clean and safe environment, including restocking supplies and performing light housekeeping duties

• Document resident care and communication with the healthcare team

• Participate in resident assessments, care planning, and implementation, under the supervision of licensed nursing staff

• Collaborate with other caregivers to ensure continuity of care and maintain effective communication with the healthcare team
