Responsibilities
Provide direct patient care to residents, including administration of medications, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Conduct routine assessments and document patient information accurately and thoroughly
Assist with daily living activities, such as bathing, dressing, and grooming
Communicate effectively with residents, families, and healthcare team members to ensure seamless care
Participate in quality improvement initiatives to optimize patient outcomes and satisfaction
