Responsibilities

• Provide direct patient care to residents, including administration of medications, wound care, and vital sign monitoring

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Conduct routine assessments and document patient information accurately and thoroughly

• Assist with daily living activities, such as bathing, dressing, and grooming

• Communicate effectively with residents, families, and healthcare team members to ensure seamless care

• Participate in quality improvement initiatives to optimize patient outcomes and satisfaction
