Onsite Community Health Navigator - Outreach and Advocacy

L.A. Care Health Plan in Los Angeles, CA, is seeking a Community Resource Center Community Health Coordinator II for onsite Pomona location.
The role focuses on promoting members’ well‑being, navigating health services, and closing care gaps through proactive, equitable whole‑person care. You will work with the Social Worker II and CRC team to engage members and visitors, provide health education, identify barriers to care, and connect them to resources, including arranging transportation and
